REH Wellness Program
Benefits:

Preventative
care will identify health issues before they
become serious problems and keep your horse
safe from preventable infectious diseases

This program
is specifically designed to ultimately and
effectively meet the needs of all horses
(horse must be at least 12 months of age to
enroll).

One annual
fee will cover the cost of exams,
vaccinations, dental, Coggins test and
deworming.
-
A detailed
report card is provided twice yearly after
each exam so you have a record of what your
horse has been given and the results of his
lameness/general exam.

Rotational
dewormers are included in program
-
Once yearly
advanced dental with sedation is included
-
You don’t
have to worry about hauling your horse to
the clinic- we will come to your farm.

At
Reata Equine Hospital our mission has always been to
provide your horses with the highest quality of health
care necessary to maintain optimal health and
performance. In continuing with this mission we are
excited to offer horse owners a new option to ensure
their horses are properly cared for year round. The REH
Wellness Program contains all of the basic preventative
care services we recommend at a greatly reduced cost to
you. This comprehensive package includes physical exams
with health reports, vaccinations, parasite control,
dental care, Coggins, nutritional counseling and call
charges. We believe this program, along with good
nutrition and regular exercise, will help you keep your
horse healthy and happy as well as improve performance.

The Reata Equine Hospital Equine Wellness Program was
created to maintain your horse in peak condition and
minimize the change of unexpected, life-threatening, and
costly illnesses. Twice yearly medical evaluations and
preventative care services are provided for your horse
by our team of specialized veterinarians.

for their vaccinations, deworming, and dental care.
Additionally, it gives you the peace of mind that comes
with knowing that you are doing everything you can to

| DEWORMING
SCHEDULE: You will receive a box of
paste dewormers labeled with administration
instructions

2011 PRICING:
- $487.00/ horse (1 to 2 horses)
- $451.00/horse (3 or more horses)

Program Details:
Our office will contact you twice yearly,
approximately 2-3 weeks prior to your
appointment, one of our veterinarians will then
visit, examine, and provide the scheduled
routine care to your horse(s) each spring and
fall. A yearly supply of deworming products will
be brought to you labeled and ready to use. All
appointments are scheduled according to
geographic location; however, every attempt is
made to accommodate client schedules.
- Additional discounts given when 3 or more horses
enrolled by the same owner
- Fee for enrollment includes listed services, 2
call charges, and dewormers for entire year
- Horses must be over 1 year old to participate in
the EWP
- Broodmares are eligible to participate but will
require additional charges for extra visits and
vaccinations (Pneumabort K) not included in EWP.

- Farm calls > 50 miles or with special scheduling
requirements may incur additional call charges at
50% of normal rates.
- Any horse never previously vaccinated will
require additional boosters at a separate visit is
not included in the WP
- Additional vaccines and preventative health
services may be recommended based on the history,
stabling, and/or show regimen of each individual
horse. Performance of these additional services will
incur additional charges. (Please request additional
services during appointment scheduling so the proper
time allotment can be made).
- Program renewal date will be on the anniversary
of original enrollment.
- No refunds or substitutions for unused services
- Prorated refunds will be made as a credit to
account if an enrolled horse dies.
- Remaining services will transfer to the new
owner if horse is sold (please notify REH if this
occurs).
- Non-current accounts may not enroll horses in
the EWP without paying account in full prior to
enrollment.
- All remaining services will be put on hold for
any accounts which become past due until account is
made current
- All incentives or additional discounts
associated with this program require accounts to
remain in good standing to stay in effect.
